Comparison of Operative Methods between Retrograde and Antegrade Nailing for Ipsilateral Femoral Shaft and Neck Fracture

Abstract
Purpose: To com pare retrospectively the antegrade and retrograde nailing in the m anagem ent of ipsilateral fem oral neck and shaft fractures.

M aterials and M ethods: Thirty-two patients (thirty-three injuries) w ere included in this study. M ean age of patients w as 38 years-old in the antegrade nailing group (16 injuries) and 44 years-old in the retrograde nailing group (17 injuries).W e com pared the union of fractures and com plications between two groups, and investigated the influencing factors.

Results: Fem oral shaft fracture was united in 10 cases (63% ) of antegrade group and 12 cases (71% ) of retrograde group, at 28.2 and 27.3 w eeks respectively. N onunion w as m ore prevalent in W inquist-Hansen III and IV (5 in antegrade nailing, 3 in retrograde nailing) than Iand II.Fem oralneck fracture w as united with 1 case ofnonunion in each group.N onunion developed from G arden stage IV, but fractures of G arden stage I and II united regardless of m ethods.

Conclusion: In ipsilateral fem oral neck and shaft fractures, the kinds of m ethods did not affect the results of shaft fractures. M inim ally displaced neck fractures also w ere not influenced by kinds of m ethods, but retrograde nailing m ay have a benefit in fixing the displaced neck fractures.
